ข้อไหล่ จริงๆ แล้ว หมายถึง ข้อสองข้อ บริเวณไหล่
- ข้อระหว่างหัวกระดูกต้นแขนกับเบ้าของกระดูกสะบัก (Glenohumeral Joint)
- ข้อระหว่างปลายกระดูกไหปลาร้ากับกระดูกสะบัก (Acromioclavicular Joint)
แต่ในที่นี้จะกล่าวเฉพาะภาวะข้อไหล่เสื่อมของข้อ Glenohumeral Joint
ข้อเสื่อม หมายถึง ภาวะที่กระดูกอ่อนของข้อซึ่งจะปกคลุมทั้งในส่วนของหัวกระดูกและเบ้าเกิดการเสื่อมเสียหาย ทำให้ช่องว่างในข้อแคบลง จนในที่สุดกระดูกทั้งสองด้านก็จะมาเสียดสีกัน และเกิดกระดูกงอก (Bony Spurs) ขึ้นรอบๆ ข้อ
สาเหตุของข้อไหล่เสื่อม
- Degenerative Osteoarthritis เป็นข้อไหล่เสื่อมตามอายุการใช้งาน ซึ่งส่วนใหญ่มักจะพบการเสื่อมในคนไข้อายุมากกว่า 60 ปี ที่เคยใช้งานข้อไหล่หนักมาก่อน
- Inflammatory Osteoarthritis ภาวะข้ออักเสบชนิดต่างๆ เช่น รูมาตอย์, โรคเกาท์, โรคลูบัส, ฯลฯ จะเกิดการอักเสบเรื้อรังของเยื่อหุ้มข้อ ทำให้เยื้อหุ้มข้อหนาตัวขึ้นแล้วไปเกาะติดกระดูกอ่อนผิวข้อ จนทำให้กระดูกอ่อนผิวข้อเสื่อมกลายเป็นภาวะข้อไหล่เสื่อมในที่สุด ซึ่งโรคข้ออักเสบที่ทำให้เกิดภาวะข้อไหล่เสื่อมมากที่สุดคือโรครูมาตอย์ (Rheumatoid Arthritis)
- Post Traumatic Osteoarthritis ภาวการณ์บาดเจ็บต่อข้อไหล่ชนิดต่างๆ ไม่ว่าเป็นกระดูกหัก ผิวข้อแตกยุบ หรือข้อไหล่หลุด ก็จะเป็นเหตุทำให้เกิดข้อไหล่เสื่อมได้เร็วกว่าปกติ
- Rotator Cuff Arthropathy กรณีที่เอ็นหมุนรอบข้อไหล่ (Rotator Cuff) ฉีกขาดรุนแรงเป็นเวลานานก็จะเป็นเหตุทำให้เกิดข้อไหล่เสื่อม เนื่องจากเมื่อเอ็นรอบข้อไหล่ขาดขนาดใหญ่ หัวกระดูกก็จะเลื่อนขึ้นด้านบนและไม่ได้อยู่กลางเบ้า เวลามีการเคลื่อนไหวข้อไหล่ก็จะเกิดการขบกันของหัวกระดูกกับขอบเบ้าด้านบน ทำให้ผิวข้อเสื่อม อีกทั้งน้ำไขข้อก็จะรั่วออกนอกข้อ เมื่อขาดน้ำเลี้ยงในข้อก็จะทำให้กระดูกอ่อนผิวข้อเสื่อมได้เร็ว จึงเป็นเหตุเร่งให้เกิดข้อไหล่เสื่อมในที่สุด
อาการของข้อไหล่เสื่อม (Symptoms)
- อาการปวด (Pain) เป็นอาการที่พบได้ชัดเจนที่สุด คนไข้มักมีอาการปวดเวลาเคลื่อนไหวข้อไหล่ เช่น ยกแขน, กางแขนเอื้อมหยิบของ, หมุนแขน เป็นต้น และเมื่อข้อเสื่อมเป็นมากขึ้นก็จะมีอาการปวดได้แม้จะอยู่นิ่งๆ เช่น เวลานอนก็ปวด นอนทับไหล่ก็ปวด
- เคลื่อนไหวข้อได้ลดลง (Limited Motion) หรือไหล่ติดยึด คนไข้จะไม่สามารถเคลื่อนไหวแขนไปได้สุดเหมือนคนปกติ เช่น ไม่สามารถหวีผมเอง,ไม่สามาถเอื่อมไปหยิบของที่ชั้นวางของ อาการไหล่ติดยึดจะเป็นมากโดยเฉพาะหลังจากที่ไม่ได้เคลื่อนไหวไหล่เป็นระยะเวลาหนึ่ง เช่นพึ่งตื่นนอนไหล่จะติดยึดมาก
- กล้ามเนื้ออ่อนแรง (Weakness) เมื่อมีข้อเสื่อมการเคลื่อนไหวลดลง กล้ามเนื้อโดยรอบก็จะลีบและอ่อนแรง หรือกรณีที่ข้อไหล่เสื่อมจากเอ็นหมุนรอบข้อไหล่ฉีกขาดรุนแรงเรื้อรัง ก็จะมีอาการอ่อนแรงอย่างชัดเจน
- มีเสียงในข้อ (Crepitus) อาจจะมีเสียงในข้อเวลาเคลื่อนไหวข้อ
การวินิจฉัย (Diagnosis)
- ประวัติ แพทย์จะซักประวัติโรคข้ออักเสบในส่วนอื่นๆซึ่งอาจเกี่ยวข้องกับโรครูมาตอย์ ประวัติการบาดเจ็บบริเวณข้อไหล่ไม่จะเป็นข้อไหล่หลุดหรือแตกหัก ประวัติการรักษาเอ็นหมุนรอบข้อไหล่อักเสบ และประวัติการใช้งานข้อไหล่หนัก เช่น ยกของหนัก หรือต้องใช้แรงแขนค่อนข้างมากเป็นเวลานาน เป็นต้น
- การตรวจร่างกาย แพทย์จะประเมินอาการปวดของคนไข้ โดยพิจารณาตำแหน่งที่ปวด อาการปวดจากการเคลื่อนไหวหรือการกด ประเมินระยะการเคลื่อนไหวของข้อไหล่ว่ามีไหล่ติดหรือไม่ มีการอ่อนแรงหรือไม่ ประเมินพยาธิสภาพของอวัยวะโดยรอบข้อไหล่ เช่น เอ็นต่างๆโดยรอบข้อไหล่ ข้อและกระดูกที่อยู่โดยรอบ เป็นต้น
- เอกซเรย์ จะสามารถบอกระดับความรุนแรงของการเสื่อมได้ โดยทั่วไปจะพบว่าช่องในข้อไหล่แคบลง ผิวข้อขรุขระ มีกระดูกงอกหรือพบเศษกระดูกหลุดลอยในข้อ เป็นต้น
- การตรวจอื่นๆ เช่น การตรวจเลือด, ตรวจน้ำไขข้อ, ตรวจเอกซเรย์คอมพิวเตอร์ (MRI, CT) จะช่วยวินิจฉัยหาสาเหตุของข้อเสื่อม รวมถึงช่วยประเมินสภาพของกระดูกข้อไหล่ทั้งหัวกระดูกและเบ้า
การรักษา
การรักษาข้อไหล่เสื่อมก็คล้ายๆกับข้อเสื่อมอื่นๆ คือ เริ่มจากการรักษาแบบประคับประคอง หรือไม่ผ่าตัด (Nonsurgical Treatment) แต่ถ้าไม่ได้ผลจึงจะพิจารณาผ่าตัดซึ่งก็มีหลายวิธีและเหมาะสมกับคนไข้แตกต่างกัน
การรักษาแบบไม่ผ่าตัด (Nonsurgical Treatment)
- Rest and Activity Restrictions: พักการใช้แขนข้างที่มีปัญหา รวมทั้งการปรับเปลี่ยนพฤติกรรมการใช้ข้อไหล่ในชีวิตประจำวัน เช่น ใส่เสื้อที่เป็นกระดุมหรือซิบทางด้านหน้าแทนการใส่เสื้อแบบสวมหัว เพื่อหลีกเลี่ยงการที่ต้องยกแขนสูง จัดวางของที่ต้องใช้ประจำอยู่ในระดับที่ไม่ต้องเอื้อมแขนสูง หลีกเลี่ยงกิจกรรมที่ต้องกางแขนเป็นเวลานาน เช่น ใช้เครื่องเป่าผม, การเอื้อมจับราวบนรถประจำทางหรือรถไฟ, หลีกเลี่ยงการหิ้ว ยก ดึง และดันของหนัก และหลีกเลี่ยงกิจกรรมใดๆที่ทำให้เกิดอาการปวดไหล่
- Cold Application: ประคบด้วยความเย็นประมาณ 20 นาที วันละ 2-3 ครั้ง เพื่อลดอาการปวดและอักเสบ ส่วนการประคบด้วยความร้อนจะสามารถลดอาการปวดและการตึงยึดของกล้ามเนื้อได้ แต่จะไม่ลดอาการอักเสบ
- Physical Therapy and Shoulder Exercise: การทำกายภาพและบริหารข้อไหล่ตามที่แพทย์แนะนำ โดยจะบริหารทั้งในส่วนของการเพิ่มความยืดหยุ่นและการเคลื่อนไหวของข้อไหล่ และเพิ่มความแข็งแรงของกล้ามเนื้อโดยรอบข้อไหล่ ซึ่งการบริหารต่างๆเหล่านี้จะทำให้เกิดการอ่อนล้าและตึงบริเวณข้อไหล่และต้นแขน แต่จะต้องไม่ทำให้เกิดอาการปวด
- 3.1. Stretching and ROM Exercise: ได้แก่
Pendulum Exercise: โดยการโน้มตัวไปด้านหน้าเล็กน้อย ทิ้งน้ำหนักแขนลงแล้วค่อยๆหมุนแขนเป็นวงกลม จากวงเล็กแล้วค่อยๆขยายเป็นวงใหญ่ขึ้น ทำประมาณ 20 รอบ จากนั้นก็กลับทิศทางการหมุน บางครั้งอาจถือน้ำหนักเบาๆ ที่มือขณะหมุนแขนก็ได้
Shoulder Circles: โดยการนั่งหรือยืนตรง โน้มไหล่ไปด้านหน้าทั้ง 2 ข้าง แล้วยกไหล่ขึ้น และหมุนไหล่ไปด้านหลังคล้ายการหมุนเป็นวงกลม ทำประมาณ 20 รอบ จากนั้นก็กลับทิศทางการหมุน - 3.2 Strengthening Exercise: เป็นการบริหารเพื่อเพิ่มความแข็งแรงของกล้ามเนื้อโดยรอบข้อไหล่ในทิศทางต่างๆเช่น หมุนไหล่ออกด้านนอก (External Rotation) หมุนไหล่เข้าข้างใน (Internal Rotation) ยกแขนไปด้านหน้า (Forward Flexion) เหยียดแขนไปด้านหลัง (Extension) และกางแขน (Abduction)
- 3.1. Stretching and ROM Exercise: ได้แก่
- Oral Medication: ยากินเพื่อลดอาการปวดและอักเสบได้แก่ NSAIDS, Aspirin ส่วนยาบำรุงผิวข้อ (Glucosamine, Chondroitin) ซึ่งแม้บางการวิจัยรายงานว่าสามารถลดอาการปวดและ ชะลอการเสื่อมได้ แต่ก็ยังไม่มีข้อสรุปและเป็นที่ยอมรับโดยทั่วไปว่าใช้ได้ผลจริง นอกจากนี้ในกรณีที่เป็นโรคข้อรูมาตอย์ด้วย แพทย์ก็จะพิจารณาให้ยาเฉพาะในการรักษารูมาตอย์
- Injection: ยาฉีดได้แก่
- 5.1. Steroid: การฉีดสเตอรอย์เข้าข้อไหล่จะช่วยลดอาการอักเสบของข้อไหล่ได้ดี ทำให้อาการปวดลดลง ซึ่งจะได้ผลดีในกรณีที่ข้ออักเสบมาก เช่น โรครูมาตอย์ และกรณีที่ปวดเรื้อรังมานาน แต่จะได้ผลไม่ดีนักในกรณีที่เป็นผู้ป่วยอายุน้อยและมีกิจกรรมใช้ข้อไหล่มาก ส่วนระยะเวลาของการลดอาการปวดก็ไม่ชัดเจน ส่วนใหญ่จะได้ผลในระยะประมาณ 3-6 เดือน
- 5.2. Hyaluronic acid การฉีดน้ำหล่อเลี้ยงเข้าข้อไหล่ มีหลายการศึกษาพบว่าสามารถลดอาการปวดได้ในระยะสั้นประมาณ 6 เดือน ส่วนในระยะยาวผลยังไม่ชัดเจน นอกจากนี้จำนวนครั้งของการฉีดและข้อบ่งชี้ในการฉีดยาประเภทนี้ก็ยังไม่ชัดเจนเช่นกัน
การผ่าตัด (Surgical Treatment)
ถ้าการรักษาแบบประคับประคองไม่ดีขึ้นก็จะพิจารณาการรักษาโดยการผ่าตัด การผ่าตัดรักษาข้อไหล่เสื่อมมีหลายวิธี ขึ้นอยู่กับอายุของคนไข้, สาเหตุโรคของข้อไหล่เสื่อม และระดับความรุนแรงของการเสื่อม
การผ่าตัดแบบไม่เปลี่ยนข้อไหล่เทียม (Non Prosthetic Procedures)
- การส่องกล้องล้างข้อ (Arthroscopic Debridement)
การส่องกล้องจะใช้เครื่องมือเข้าไปตัดแต่งผิวข้อที่เสื่อมและล้างสารตกค้างในข้อที่ทำให้เกิดอาการปวดอักเสบออกมา, เข้าไปตัดเยื้อหุ้มข้อที่หดรัด, เจียรแต่งกระดูกงอก, แก้ปัญหาการกดทับเอ็นที่อยู่เหนือข้อไหล่ ทั้งนี้ก็เพื่อเป็นการลดอาการปวดและเพิ่มการใช้งานของข้อไหล่ ซึ่งถือเป็นการซื้อเวลาก่อนที่จะต้องผ่าตัดเปลี่ยนข้อไหล่เทียม ผลของการรักษาด้วยวิธีนี้มักจะได้ผลดีในกรณีที่ข้อเสื่อมไม่มาก คนไข้อายุน้อยกว่า 60 ปี อย่างไรก็แล้วแต่อาการที่ดีขึ้นก็มักจะอยู่ได้ประมาณ 2 ปี แต่พอเกิน 4-5 ปี ก็มักจะกลับมาปวดเช่นเดิม - การเจาะฐานกระดูกอ่อนผิวข้อ (Microfracture)
เป็นการเจาะรูบริเวณฐานกระดูกอ่อนผิวข้อที่มีความเสื่อม เพื่อหวังผลให้สเตมเซลล์ (Stem Cell) ที่อยู่ในโพรงกระดูกเคลื่อนขึ้นมาปกคลุมบริเวณผิวข้อที่เสื่อมแล้วแปลงเป็นเนื้อเยื่อที่คล้ายกระดูกอ่อนผิวข้อ ซึ่งจะช่วยลดอาการปวดและเพิ่มประสิทธิภาพการทำงานของข้อไหล่ได้ โดยผลของการรักษาด้วยวิธีนี้จะได้ผลดีในกรณีที่ผิวข้อเสียหายมีขนาดเล็กและผู้ป่วยอายุไม่มาก อย่างไรก็แล้วแต่การลดอาการก็จะได้ผลในช่วง 2-4 ปีแรก หลังจากนั้นก็จะกลับไปมีอาการเช่นเดิม - การปลูกถ่ายเนื้อเยื่อ (Osteochondral Autograft/Allograft Transplantation), การปลูกถ่ายเซลล์ (Autologous Chondrocyte Implantation)
เป็นการนำเนื้อเยื่อกระดูกและกระดูกอ่อนผิวข้อ หรือนำเซลล์กระดูกอ่อนที่ผ่านการเพาะเลี้ยง มาใส่บริเวณผิวข้อที่เสียหายเพื่อให้ผิวข้อถูกปกคลุมด้วยกระดูกอ่อนผิวข้อโดยตรง ในปัจจุบันยังมีการศึกษาเกี่ยวกับการรักษาด้วยวิธีดังกล่าวในข้อไหล่ค่อนข้างน้อย แม้ว่าบางรายงานจะได้ผลดีในระยะสั้น แต่ก็ยังไม่สามารถยืนยันผลในระยะยาวได้ ดังนั้นจึงจำเป็นต้องมีการศึกษาวิจัยให้มากขึ้น - การเย็บปกคลุมผิวเบ้าด้วยเนื้อเยื่อ (Biological Interpositional Arthroplasty)
เป็นการเย็บแผ่นเนื้อเยื่อให้ไปปิดผิวเบ้าที่เสื่อม เนื้อเยื่อที่ใช้เย็บปิดมีหลายชนิดได้แก่ เอ็นส้นเท้า, หมอนรองเข่า, เยื่อหุ้มข้อ เป็นต้น เนื้อเยื่อเหล่านี้บางชนิดก็ใช้ของผู้ป่วยเอง (Autograft) บางชนิดก็เป็นของผู้อื่น (Allograft) การผ่าตัดแบบนี้จะใช้ในกรณีที่ข้อไหล่เสื่อมทั้งในส่วนของหัวกระดูกข้อไหล่และเบ้า แต่ยังไม่ต้องการที่จะผ่าเปลี่ยนในส่วนของเบ้าเทียม เช่น ในกรณีที่ผู้ป่วยอายุน้อยกว่า 50 ปี ส่วนผลก็จะดีเฉพาะในการเลือกใช้เนื้อเยื่อบางประเภท เท่านั้น และมักได้ผลในระยะสั้น ผลในระยะยาวยังไม่ทราบชัดเจน - การเจียรผิวเบ้า (Ream and Run Technique)
เป็นการเจียรผิวเบ้าเพื่อให้ผิวเบ้าเรียบและเกิดความโค้งที่เหมาะสมรับกับหัวกระดูกหรือหัวข้อเทียม ก็เป็นอีกวิธีที่ต้องการหลีกเลี่ยงการผ่าตัดเปลี่ยนข้อเทียมในส่วนของเบ้า เช่น ในคนไข้อายุน้อย ผลการรักษาก็มีแต่รายงานว่าได้ผลดีในระยะสั้น ยังไม่ทราบผลในระยะยาว อีกทั้งการศึกษาการรักษาวิธีนี้ก็ยังมีน้อยอยู่
การผ่าตัดเปลี่ยนข้อไหล่เทียม (Shoulder Replacement)
มีหลายแบบดังนี้
1. Stemless Hemiarthroplasty (Surface Replacement Arthroplasty)
เป็นการผ่าตัดเปลี่ยนข้อเทียมเฉพาะในส่วนของหัวกระดูกข้อไหล่ โดยจะเจียรส่วนของผิวข้อที่เสื่อมออก แต่จะไม่ได้ตัดทั้งหัวกระดูก แล้วจึงแทนที่ด้วยข้อเทียมที่เป็นโลหะปกคลุมหัวกระดูก โดยจะไม่มีด้ามเสียบลงไปในโพรงกระดูกต้นแขน การผ่าตัดวิธีนี้ก็เพื่อเป็นการอนุรักษ์มวลกระดูกบริเวณหัวกระดูกไว้ จะเหมาะสำหรับคนไข้อายุไม่มาก มีการเสื่อมของหัวกระดูกไม่มาก และเนื้อกระดูกยังดีไม่พรุน จากการศึกษาพบว่าได้ผลดีทั้งในระยะกลางและระยะยาวในกรณีที่เป็นข้อไหล่เสื่อมจากอายุการใช้งาน และโรคข้ออักเสบรูมาตอย์ โดยระยะการคงอยู่ของข้อเทียมประมาณ 96% ที่ 20 ปี
นอกจากนี้ยังมีการผ่าตัดเปลี่ยนผิวข้อเทียมเพียงบางส่วน (Anatomic Focal Resurfacing) แต่มีที่ใช้น้อยในการรักษาข้อไหล่เสื่อม เนื่องจากข้อไหล่เสื่อมมักจะเสื่อมมากทั้งหัวกระดูก
2. Stemmed Hemiarthroplasty
เป็นการผ่าตัดเปลี่ยนข้อเทียมเฉพาะในส่วนของหัวกระดูกข้อไหล่ โดยใช้ข้อเทียมที่เป็นโลหะทรงกลมแทนหัวกระดูกและมีด้ามเสียบเข้าไปในโพรงกระดูกต้นแขน ซึ่งจะยึดด้ามกับโพรงกระดูกด้วยซีเมนต์หรือไม่ก็ได้ แต่การผ่าตัดประเภทนี้จะไม่มีการเปลี่ยนข้อเทียมในส่วนของเบ้า มักจะใช้กรณีที่เบ้ายังปกติดีอยู่, กรณีที่เบ้าเสียหายรุนแรงจนไม่อาจที่จะใส่เบ้าเทียมได้ กรณีที่เป็นข้อไหล่เสื่อมจากเอ็นหมุนข้อไหล่ฉีกขาดรุนแรงเรื้อรัง เป็นต้น ผลการผ่าตัดด้วยข้อเทียมชนิดนี้จะได้ผลดีในระยะกลาง(ประมาณ 10 ปี) แต่ในระยะยาวจะมีโอกาสเกิดการสึกกร่อนของเบ้าตามมาช่องว่างในข้อแคบลง และผลของรักษาก็จะลดลงตามเวลา โดยระยะการคงอยู่ของข้อเทียมประมาณ 82% ที่10ปี และ75%ที่20ปี
3. Total Shoulder Arthroplasty
เป็นการผ่าตัดเปลี่ยนข้อเทียมทั้งในส่วนของหัวกระดูกและเบ้า ซึ่งจะมีข้อเทียม 2 ประเภท
3.1 Conventional Total Shoulder Arthroplasty
เป็นการผ่าตัดเปลี่ยนข้อเทียม โดยด้านหัวกระดูกเป็นข้อเทียมโลหะทรงกลมที่มีด้ามเสียบเข้าไปในโพรงกระดูกต้นแขนเหมือนกับของประเภท Hemiarthroplasty แต่ส่วนของเบ้าจะเป็นข้อเทียมพลาสติกซึ่งยึดติดกับฐานเบ้าเดิมโดยใช้ซีเมนต์
การผ่าตัดประเภทนี้เป็นการผ่าตัดซึ่งใช้บ่อยที่สุดและได้ผลดีในการรักษาข้อไหล่เสื่อมในผู้สูงอายุมากกว่า 60 ปี มีการศึกษาเปรียบเทียบการใช้ข้อเทียมแบบ Total Shoulder Arthroplasty กับข้อเทียมแบบ Hemiarthroplasty ในการรักษาข้อไหล่เสื่อมตามวัยในผู้สูงอายุ พบว่าข้อแบบ Total Shoulder Arthroplasty ได้ผลดีกว่าทั้งในแง่ของการลดอาการปวด การเพิ่มการใช้งานของข้อไหล่และความคุ้มค่า โดยระยะการคงอยู่ของข้อเทียมประมาณ 90-95% ที่ 10 ปี และ 80-85% ที่ 20 ปี
3.2 Reverse Total Shoulder Arthroplasty
