Avulsion injury of distal biceps is relatively rare. Once it occurs, surgical treatment is usually mandatory due to the consequence if it is left untreated. The surgical repair has provided good functional outcomes. There are variety of surgical techniques. We present a novel technique for anatomic reattach of the distal biceps that allows to reinsert distal end of the tendon into the bone tunnel using tension sliding technique and biotenodesis screw. The surgical steps are simple, reliable, safe and efficient.
